Title: Homeless Outreach Care Coordinator

Synopsis/Description: Serves as a member of an interdisciplinary team that provides recovery oriented in- home and community based services for individuals who have experienced a crisis, exhibit functional deficits in self-management, maintaining treatment, and in activities of daily living/community living skills. Care Coordinator will provide comprehensive care coordination services for adults and children, health promotion, individual and family support, referrals to community agencies, assessments, treatment planning and access to Habilitation/Child Mental Health Waiver services if appropriate. The Care Coordinator delivers intense care coordination services for 30 days post crisis presentation to connect to long term services and supports.

Title: Crisis Stabilization Therapist

Synopsis/Description: The Therapist of Crisis Stabilization will be responsible for preparing psychosocial assessments of individuals and families as well as serving as primary therapist in providing crisis treatment including leading individual and/or focused group therapy. The therapist provides counseling and therapy services to clients that halt, control, or reverse undue stress and social, emotional, or behavioral problems that threaten or have negatively affected the individual’s or family’s stability. Activities can include individual, family, or group therapy, crisis intervention, trainings and marketing services. The Crisis Stabilization Therapist must possess and exhibit exceptional judgment, excellent verbal and written communication skills, and the ability to supervise and lead a team. This position is included in a rotating on call rotation that is required. These responsibilities are carried out under the direction of the Supervisor of Clinical Residential.

The Crisis Stabilization Residential Youth Advocate ensures the safety and security of the children and youth at all times and assists them in the performance of regular daily activities, including bed making, personal hygiene, room cleaning, and all other chores normally expected of children and youth their ages. The Crisis Stabilization Residential Youth Advocate provides a caring, warm, consistent environment for the children through setting clear boundaries, supporting, encouraging, and giving verbal praise and acknowledgment for positive actions and decisions to help build self-esteem, age-appropriate interpersonal skills, positive thinking, problem-solving, coping skills, respect for others and self and brainstorming alternative, appropriate behaviors, etc. The Crisis Stabilization Residential Youth Advocate will complete accurate and timely documentation as required, provide direct care coverage, and participate in an on-call schedule. The Crisis Stabilization Residential Youth Advocate will participate in Crisis Stabilization staff on-call rotation and report for assigned on-call shifts when called upon. This position will need to have a flexible schedule that may include working some nights, weekends, and holidays.

Synopsis/Description: As a Behavioral Health Care Coordinator, for Hillcrest Family Service’s MHC-Dubuque County, your primary responsibilities include working within the Eastern Iowa Region completing mental health screens and assessments for individuals who are incarcerated and/or involved with the court system.
